No reason was ever found.
However, moving the group to the secondary node and rebooting the primary node, and moving the groups back resolved the issue.

Good Question, bad answer options.
Option A is also correct. Setting the fillfactor = 0 sets it to the default which is set to 100% at installation.

Tried to initiate a failure. That did not work.
All other resources in the group are online. Users can even connect to the database and run queies.
