If you have another server, try restoring the database there and then back it again to compare the differences in time between the two servers. That should help rule out...

Have you checked the Windows Event logs to see if you have a hardware error? It could be as simple as a bad disk that is causing the degredation.

Sorry to say we never got to the bottom of it. After spending a fair amount of time on it we decided to continue using SQL Authentication.

Don't install SP4 as you must have AWE configured to be addressing 8GB of RAM. Read KB899761 for a description of the problem.
