(All done by a maintenance plan.)
10:00pm Full Backup
10:15pm Transaction Log Backup
10:18pm Did some inserts
10:30pm Transaction Log backup

yep. I just tried the Rebuild.exe and it failed. I am in the process of re-installing SQL Server now.

They are 2 different servers. The Standby is off-site as I stated above.
