Instructions slightly different for me because I'm using SSMS 2014

Close SSMS

Go To

%APPDATA%\Roaming\Microsoft\Microsoft SQL Server\120\Tools\Shell

delete the RegSrvr.xml file

open SSMS

Fixed!

As a side note... the previous RegSrvr.xml and the new.... are completely identical,...
