Did you check whether your package is maybe set to 64 bits?
64 bits won't debug as far as I know.

Have you got a boolean (yes/no) field in it?
May sound weird but make sure it has a default value of 0.
I would say: Test without the boolean field: See if...

Another lesson learned!
However:
The crux is not in the decimal(,xy) but in the 1.0!
(Sum([PremiumOnTime])*1.0/Sum([premium]) )
Just putting in the 1.0 makes that I get all the decimals that I could think of.
