I would do a profiler trace of the DB activity during the time of occurance. Any number of things could be happening from common locking to more obscure problems like iterative recompilation...

If the rule is as simple as always adding 4 zeros to the front of an existing column, why not use a view instead of storing this information?

yes there are
although the pk on the parent isn't clustered
when I overlayed a non-clustered index on top of the ci, performance improved and was similar to the manual deletes performed...

On thinking about this could cascade delete be doing record based operations vs set based operations? (since my own constructed deletes would be using set based ops)

Also, not knowing the query which is being submitted....
The Set FORCEPLAN may be conflicting with your query hint (which is what the error message appears to be telling you)

Not sure if you have tried running the index wizard on this, if so the wizard can create some statistics which do not always get deleted unless it is completed....
