    Are you looking for a form to give to users so they can request a DB?

    I don't have one handy, but we used to ask for size(data), guess at transactions to get log growth. Guess of data growth. Recovery needed( once a day, within an hour, etc), # users, type of app, contacts.

    From there we could determine if it could go on an existing server or we would buy a new one, charge backs, etc.
