If I do a reverse engineering from visio to sql server DB, the changes made at visio does it effect the connecting DB?

No it doesn't.

Unless you want it to. There is a way in visio to refresh model, and you can either push the changes to the server, or revert back to the old model.

This is now restricted to very specific versions of visio (I think the one that comes with VS2005 Team Edition). VS Pro no longer has the ability to push changes back to SQL.
