Currently, we are experiencing time outs with our Veritas Server connecting to our SQL Server databases. In the sql logs I see:
BackupMedium::ReportIoError: write failure on backup device 'VNBU0-561-584'. Operating system error 995(The I/O operation has been aborted because of either a thread exit or an application request.)
OR
BackupVirtualDeviceFile::RequestDurableMedia: Flush failure on backup device 'VNBU0-561-584'. Operating system error 995(The I/O operation has been aborted because of either a thread exit or an application request.).
Has anybody come across this before? -JG

We experience this qquite often. The message you described occurs when , for whatever reason, when a database Backup to Veritas fails. Some of the problems I have experienced are as follows:
1. The Backup times out due to a loss in connection to the master server
2. Bad tape
3. Bad drive
4. Problems on Silo
5. Class not active
6. Misconfigured .bch file
One way to troubleshoot this error is to log on to the media server where the NetBackup MS SQL Client software is installed, and Load the Netbackup Database Extension GUI - Click on View, hit F9 (or Monitor Jobs) , click on Verbose, and you should see the og for the failed job (s)...If you read this carefully, it should help you determine what caused the error.
