Tool to manage & track Datapatches and Schema changes

Hi
Can any body has any tools to apply datapatches (INSERTS,UPDATES) and ERchanges to automate the process of applying them using query analyzer.
as we can do so by using OSQL/ISQL , but If anybody has any idea about some other tools to manage/track the patches and Schema changes..

The tool is better working with Schemas ,but If try to include data too in the package its taking lot of time and resources especially lot of space on "C:\" drive(Creating temp folder and placing temp scripts in it and we usually have less space on C
... 
Overall The tool is very much useful when we want to package only the Schema (ER) and Schema Changes of the database

You might also want to look at the Packager Plus product from Innovartis
(http://www.innovartis.co.uk). It can package your data and schema and dynamically update the target database at install time which means it can account for variations in the target schema and data seamlessly.
