Are there any templates for documenting a SQL Database? I have been trying to document my databases using MS Word and would like to know from Sr DBA's what is the correct procedure and if there are any templates. Any advice or suggestions will be greatly appreciated.

Search the scripts database, there are a couple of very good scripts for generating documentation of your database.
Also, you may want to check out http://www.ag-software.com/. They have a sql documentation tool that will generate a compiled help file. The documentation it produces is very thorough.
