There's more options than that in the section in the book chapter, especially if you're on SQL 2008... Also, have you checked the cache size, confirmed the wait type, etc?

Continue to monitor CMEMTHREAD over time and watch how fast it grows.
CMEMTHREAD is the name of the wait type, not the cache. The cache name is TokenAndPermUserStore.
