We are having a problem that I cannot figure out. We have setup the proxy account for Non-SysAdmins to run jobs using xp_cmdshell and it has been working fine.
Within the last month or so we have started having problems where the EXEC statement on xp_cmdshell will fail. I can go back in and reset the proxy account password and everything works fine. This isn't a case of a password getting locked or expiring because when it happens, I just reset the password back to what it was before and everything works fine.
I have checked all the logs I can find and I can't find any entries related to this happening. The problem is that I don't even know exactly when it is happening because it may be a while before a job that uses it even runs. I have checked over the full time periods between the last successful run and the failure and I just can't find it.
I have researched this problem everywhere and I cannot find this particular problem. Like I said, I have had it setup right because it worked fine for a while, it is just all of a sudden.
If you have ever had, or heard of, this problem, I would appreciate any feedback to help me figure this out.
Just FYI, our temporary solution is an encrypted stored procedure that runs periodically to reset the password. This has worked pretty well but we have still had one (maybe two) problems even while doing this.

At the server level (OS), what security auditing do you have turned on? If you have Success and Failure specified on Audit account logon events and Audit logon events (in the Local Security Policy | Local Policies | Audit Policy), do you see any corresponding entries in the Security Event Log?
