Anybody know why the ApplicationName is blank on Event:SQL:BatchStarting or SQL:BatchCompleted when I run a trace in SQL Server Profiler..
The user has SQL Server 2005 Express installed and is the only user where the applicationName is blank. All others have full Management Studio installed and it is never blank.

Can you re-create the problem with another copy of Express?

Not really, he is the only one using Express and nobody else wants it installed.

Install it on a dev/test box, or a virtual machine. That's your best bet for finding out what's up.

Did find one user who was using express, had him log on and checked both users in Activity Monitor. the application name for him was in there as Microsoft SQL Server management Studio.
The user with the problem has a blank application name there also. This user just got a new pc and installed the express version, said he did not get any errors when installing, but obviously something is wrong.

Sounds like you're on the right track on that one.

Checked his laptop and he has full management studio installed. The application name does not show up in activity monitor either. Only him!
Anyone one got any ideas of why this is happening? I have found nothing.

I found the problem.
This problem was only affecting one user. the application name will be blank in Activity Monitor if setting are changed to open new query window at startup. For any new queries that are opened, the app name appears, that is why it seemed sporadic. This will not be an issue if you do not need to capture any events for audit purposes (which we do)
In SQL Management Studio select Tools, options
At Startup must be set as - Open Object Explorer
