I can't answer all of them but:
Can anyone tell me the when following availability solutions were introduced in sql server?
1. Clustering
- I never saw it in 6.0 and actually did it in 6.5, so I'm going 6.5 with Win 2K, rough I tell ya.
2. Logshipping
- I believe SQL 7 officially supported it.
3. Replication
- At least back to 6.5, and only transactional, merge and snapshot were added later, 7.0 I think.
4. Mirroring
- Not sure either.

Mirroring was 2005.
Replication might have been in 6.0 since the 6.0->6.5 upgrade was like 6 months.
Clustering was in NT 4 and v6.5. It did not work well, and was flaky. The install was something like a 60 page document.
The others sound right to me.
