Does MAPI client needs to be installed on the SQL server when configuring SQL mail.

Yes you must install a MAPI client (MS OUTLOOK) and then create an active profile. After this you should be able to configure SQL Mail.

Which service need to be started by domain account? sql server or sql agent?

I would do so with both. Depending on how the sqlmail is going to be used.
If you are just going to use sqlmail in sp's etc then just set up the domain accout for sql server. SQL Server agent would create it's own mapi connection to send out mail, so if you will be using that to send email then use a domain account for that as well.

Where to save it after wrote the details in xp_sendmail and shedule xp_sendmail.
