Hi,
The databases of my application is granted access by GeoTelAdmin. Now not sure the permission was un-checked and application started throwing errors. After checking we could find SQL permissions of GeoTelAdmin bein unchecked. After selecting the option and restart SQL services, application started working fine.
Any idea as which logs to collect to know when and who un-checked the permission. Is it User or application which did that?

SQL doesn't keep historical logs for this kind of thing by default. If you did not have a trace running at the time, there's no way to find out who did this and when
