So I started an in-place upgrade on a test box with a test instance of 2000, running SETUP.EXE from the 2005 disk. I got pretty far along without it ever asking me if I'm doing a new install or upgrading an existing instance. It seemed as though it was just doing a new install of 2005. I cancelled it. Did I get impatient ??
(Can't edit the title ... meant to say "2005" .... D'oh !)

2005 and 2000 can coexist on the same box. If you were installing the software, then, yes, you were (are?) impatient. It's only after you've installed SS 2k5 that you can then do an 'inplace' upgrade. All the libraries and tools have to be in place before you can play with data.

So, how do I start the in-place upgrade ? I thought running 2005 setup again would prompt me for an instance, but it just tells me I already have 2005 installed ...

Once you have installed an instance of SQL 2005 you can add new instances or features from Control Panel Add\Remove Programs.
