This past weekend we had some instances fail to start because SQLSERVR.EXE for SP4 was Quarantined by McAfee. McAfee came out with a fix late Friday night 3/10 but it was too late for us.
We have other servers with the file missing but the instance is still running because it has not been restarted. McAfee is able to Quarantine an executable even if it is being used. You can find the this file in the directory c:\Quarantine
The message you get when trying to start the instance is a file is missing. It would be nice if the message said what file is missing.
I thought this information maybe of some help.
