I have a SQL 2000 5 node (4 active/1 passive) cluster. Each of the node has SP4. Everything has been working well until...
There was a memory chip that was detected as in the process of failing. We failed-over the affected node and replaced the memory (one of the big advantages of SQL Clustering). However, when we restored the node I noticed that the SQL performance counters were no longer available (based on a monitoring tool that we have to watch the nodes in the clusters).
I restarted the affected node and got the following error message in the SQL Error Log:
===> Performance monitor shared memory setup failed: -1
I've read several articles (MS and other) but everything seems to be related to needing to add SP4 to the instance - not so here.
Any insights would be appreciated.

I'm having the same problem. Performance monitor is not running, but the error still appears when sql server service is restarted. OS is Windows 2000 V 5.00.2195 (sp4). I'm wondering if network admins may have applied an OS patch that's causing the problem.
Glenn, have you found a resolution yet?
