Is MS03-31 (8.0.818) hotfix cluster aware? Are there any special steps to take to apply it to an active-active cluster?

It is cluster aware. You have to install it for each instances. Make sure you have sp3 or sp3a installed prior applying it.

Allen is correct, here is some info I found on using the hotfix installer from MS:
"
Cluster Installation
SQL Server 2000 Cluster Installations
To apply the Hotfix package to a clustered SQL 2000 installation, you must run the hotfix package on the node that owns the SQL Server resource. Then, the Hotfix installer will take the SQL Server resource offline and update the binaries on all nodes of the cluster. Finally, the SQL Server Resource is brought online, and the Installer runs any install scripts (.sql) that were shipped as part of the Hotfix."
You probably don't have to but I then rebooted my nodes and it worked great.
