The only oddball one I could come up with would be that whatever space I'm staying in (hotel room, whatever) shouldn't have been recently cleaned with perfumed products, and no perfumed powders, etc., on the towels, sheets, pillowcases, etc.
Most pefumes give me a headache, so I try to avoid them.
Not sure I'd actually demand that, but it's as close as I can get to diva-hood.

Fresh-brewed coffee on demand, high-speed internet access and to please be left alone.
Last year at TechEd South Africa, the MVP lead ran a 'hunt the MVP contest' where there were prizes for getting signatures from 6 South African MVPs. Now, to put this in context at the time there were only 12 South African MVPs and only 10 of them were at TechEd. Net result, those of us who were there were getting constantly harassed all day for signatures.
If that's what fame brings, I don't want it.

I certainly don't want more fans either. The SQLSaturdays have featured a scavenger hunt card, and I've been on a few, but it hasn't been too bad. We'll see in Baton Rouge how that works out when I'm there all weekend.

Seriously, I'd demand proper A/C. I cant handle heat/humidity. I'm perfectly comfortable in shorts and T-shirt when it's 20-40F ( -7 to 4 Celsius) degrees out.
It's about 75F (24C) right now in the office, the A/C has been off all night, so everything is sort of sticky due to the humidity. The A/C turns on in about 30 minutes. It won't get any cooler, but the humidity will get cut down.
Oh, and besides my Monocle, I'd like a never ending supply of Pepperoni pizza from Lombardi's in lower Manhattan.

I'll take a pocket-size cold fusion powered electro-magnetic pulse device so that I can ensure no one is talking on a cell phone or using a Blackberry during my incredibly important and overly interesting speech.

Since this is an open-ended question and nothing is out of play, I would demand a lavish suite at the hotel. Fresh fruit (just picked, cleaned and sliced and diced), OJ, fresh ground Brazilian coffee, egg white omelette with feta, tomatoes, onions and broccoli, and wheat toast for breakfast. Dinner would be a full slab of ribs and mac n' cheese from Slows Bar BQ in Detroit (yes, they would have to fly the meal from Detroit to where ever it is I am staying). Throughout the room would be candy dishes filled with the "Fun Size" Twix, Snickers and Almond Joys unwrapped. Also, a Swedish masseuse trained in the fine art of deep tissue massage available at my beck and call. Skip the mini bar, the suite would have a full bar, stocked with the finest beers (that's a conversation in itself), as well as a few nice reds from the Old Mission Peninsula wineries in Northern Michigan.
I guess I worked too late last night and back to the grind too early this morning, my mind is wandering...
In reality all I need is a clean bed and a clean bathroom.

Here's one for GSquared and anyone else who finds the smells of "room deodorizers", heavily scented soaps, carpet cleaners, etc. irritating.
An Ionizer machine. Most major hotel chains have them and will provide one if you ask for it. The heavy duty ones have timers that you can set to run up to 1 hour at a time. If the hotel doesn't know what one is you should probably consider a different hotel.
Ionizers cause the pollutents to settle out of the air, and have saved me from waking up with splitting headaches many times.
It also helps to let the hotel know when your making the reservation or during checkin that you would prefer they not use room deodorizers during your stay.
