You'll need to read up on dimensional modelling.
The Data Warehouse Toolkit by Kimball has a specific chapter on insurance.
Star Schema The Complete Reference is also a very good book.
Need an answer? No, you need a question

If you're starting from scratch as your post implies, you might consider at least starting from any of a number of industry "standard" models, modifying them to fit your goals of course. We had to take that on a while back, and frankly using the industry models at least as a benchmark were very useful in cutting down our design time.
ACORD has done a lot in this field (ACORD information model), as well as IBM (IAA) , Informatica (Informatica insurance suite), GuideWire (Millbrook data mart), just to name a few. There are literally dozens of smaller ones as well: just pop that phrase into Google and voila.

Your lack of planning does not constitute an emergency on my part...unless you're my manager...or a director and above...or a really loud-spoken end-user..All right - what was my emergency again?
