We are trying to put SP3a on our SQL Server. Here is the problem :
The default server name is :
CATEST
And the named instance server name is :
CATEST\CATEST
When we try to put SP3a on it for some reason it does not recognise the default instance name. Any idea why ? But Thur EM I can connect to both the instances.
Any help is appreciated.

Try renaming the named instance, apply sp3a and rename again if wanted.
I think it is not wise to name an instance the same as the machine name. Specially if you already have a default instance...
When you need more than one instance, it is recommended to install all instances as named instances.

I wonder if there's a way to change the instance name (part after \) other than reinstall server with different name. Service names, directories etc. have named instance name included, so I don't think it's possible.

What operating system are you running on and what network libraries are currently configured (Named Pipes, TCP/IP, etc.)? When you're connecting from EM, is this just locally or can you connect from a remote system?

Its on a windows 2000 Server with SP4. Yes I am able to connect to both the instances from other machines thru TCP/IP as they are running on different ports.

Is the default instance listening on port 1433? Is there an alias in place that shoots it to the named instance (check by running cliconfg locally)?
