...And a plethora of newly hyped old ideas contributes a lot to software development failure: the misunderstanding of things like "extreme programming", "agile", and other rather ancient development methods (with shiny new overhyped names) and their consequent misuse does a lot of damage.
In my opinion, it is the misapplication of all these techniques (including both agile and waterfall) that causes the most issues.

Agreed. To go a bit further it is the dogmatic belief that methodology is the solution that is as false as any other assumption.
