How to setup rules or process which find out if any given SQL server has reached its capacity (CPU, Memory, Disk queue lenth, No. of database etc) or not .

I am sure this is not what you want to hear but, let me explain about what I am doing for that just in case..
There isn't any really easy way by just using SQL Server tool. It is doable to read perfmon data by using ActiveX and other stuff but not recommendable. I watch Database Size by using DTS package so that I can calculate the database growth rate but rest of them, I found 3rd party tool is a lot easier. This is one of the product I found very useful if you have a lot of sql servers as well as other servers to watch.
