I waited to update to 19.0 until SSMS Tools Pack supported it. I will wait for it to support SSMS 20.0 before I upgrade to that major version. I usually update to new point releases within a couple of weeks of release.
20.0 brings a major security change regarding database connections that may make that more complicated. Brent had a good blog post about this today: Read This Before Your Users Install SSMS 20 - Brent Ozar Unlimited®

Quite frequently. 19.x fixed a slow startup on 18.x

Typically only update SSMS when we update the actual SQL version. Most of the users have 19.3 but there are still a few 18.x instances around here...
