we're seeing the following error on QA.. any ideas what the issue could be?
Attach is a screenshot of the sys.databases log_reuse_wait_desc column

The transaction log for database " " is full. To find out why space in the log cannot be reused, see the log_reuse_wait_desc column in sys.databases
We don't take xlog backups.

Actually it's waiting for a transaction log backup to truncate the log. Is this on a Production Server ?

If you don't take TLog Backups, you can't restore the data back to the point in time in case of a disaster and TLogs will be filling up if the DB is not in Simple recovery model.
Switch the DB to simple recovery model if you don't take TLog backups. If you put it in simple, you can't have DB Mirroring or Log shipping configured and you miss the opportunity to restore point in time.
But if you still want to use Full\Bulk-Logged recovery models, you need to configure TLog backups at regular intervals.
