Thanks everyone & Sorry for such a long delay...
Cause found: It was improper set up of the SANS. We used the Microsoft's SQLIOStress utility to check the SANS sanity. Once we resolved the SANS configuration issues, the problems disappeared.
Apparently the buffer was messedup and the SQL Server was not properly seeing the identity value. As to why duplicate value was inserted in a Primary Key column is not explained properly. But they all related to the SANS.

Thanks for posting the results. Good to see it is finally solved somehow.
