I've run CONTIG against my own machine with MSDE running, and it defragments quite well even with the DB's in use. I don't know how great it is against a production DB, but it's from the SysInternals guys (so I'd imagine it would hold its own).
That being said - DisKeeper standard (or enterprise if you volumes are over a certain size - 2TB maybe?) will defragment while the server is online. Won't be perfect - but it will keep them defragged enough that the fragmentation isn't a factor (read their FAQ at some point - it's a pretty interesting read about "some fragmentation is good").
Since we put DK in - we use the offline time to run the OS volume/Master file table defrags (which occur during reboots of the server.)
