we are planning to setup a disaster recovery process on our SQL server Box.
Can I find any documents or can anyone let me know what are the steps I need to do.

The Microsoft documentation has lot of good information on the Disaster Recovery Methodologies and Backup and Restore Strategies.

I have a document showing what servers we have running SQL Server, what version of SQL Server/ Service pack, what version and service pack of operating system and what databases are on each server. Then I have detailed documents with screen shots of Installing SQL Server and the service packs with how we have them configured. Then I have detailed info with screen shots of how to recover the databases for each server, where connections come from, etc. These are reviewed every month and WORD versions are burned to CD once a month and taken offsite.

One other thing. Once a month the master list of 'sa' and other login/passwords is burned to CD once a month and taken offsite as well.
