Hi, I am looking for a word document for a monthly database health check reports.
I do not require any tool as I am already using Foglight.
I need to have some report which includes a format of general health of databases plus critical incident happened in the month plus changes like new upgrades or installations.

so what you are asking is for someone to provide you with a set of basic database/server checks in a word document for you to produce reports on something you presumably know nothing about?
To avoid making yourself look silly if you get something wrong or someone asks you a question about a check I'd suggest you buy the Administrators companion for SQL Server as I'm pretty sure it will not only cover what you want ( but probably not a word template ) but will also explain how to perform checks and interpret the results.
I'd be very suprised if SQL Central doesn't have some articles about back practice/checks/daily reports as well.
The one thing I would say about daily checks is that there is a certain amount of what is right for your environment, most people I guess have some sort of automated monitoring to cover basics, for instance I don't have to check my clusters are up ( notwithstanding the panic in the office if the system was unavailable ) because I'd have been alerted already if they were down.
What you're appearing to ask is quite a wide subject area and without a simple few lines answer.
