Can u please assist here ,
I have a scripts that alert DBA's when the manual\automatic fail over occurs in a db mirroring environment ,now i need another script or way that will show who (PERSON) did\activated fail over.

I wouldnt know off-haand how this would be acheived but you would almost certainly need to turn on a higher level of logging.
Is this a frequent problem?

Not frequently but i would love to know what user activated\resulted to the failover when it happens,please elaborate what do you mean by higher the loggins ?

I'm not sure exactly whats involved but i do know that there is a higher level 'auditing' that can be turned on. As it suggests this loggs a lot more info (not just when things go wrong).
If you monitor the logs frequently, this could generat a lot of 'noice'.
