What exactly do you mean by "database document"? If you are referring to a database diagram then you can use the diagram feature in Enterprise Manager to create one and add the tables you want to it.

for example, in software/application development, there is a specific documentation such as System Requirement Specification Documentation where we put all the requirement for a system in that document.
So, is there any documentation for database? the diagram in the sql server is common for everybody.

Which documents that need to exist depend on the processes and policies that a company is using. For instance, I normally work with agile software development methodologies and we have no such document as a "System Requirement Specification Documentation" (which is natural since we do not have any documents at all).
If you are asking for some kind of generally adopted document standard for documenting SQL Server instances then I do not know of any. And if you are asking if there is any way built into SQL Server for creating one then the answer is, naturally, no, there is not.
