That problem seem to be a disk controller issue its good to contact your vender for further assisment.

Hi - have you tried testing the IO system using the SQLIOSim tool?

have you been review eventolog from windows?
any failover?
you can review sql server dump files and look for errors..
The SAN loose disks?...SQL Server can see all disks?..
Some times when a site suffer a blackout, SAN disks are loose..and SQL Server dont see the files.
maybe this can provoke problems..
