Hi,
What i can make out of your requirement is listed below:
1. You want to secure your data.
2. You want to know who is accessing what data.
If my assumption is correct i think you should look into Security and Auditing features of your SQL Server.
If you can narrow down your requirements maybe i can be of more help.

Maybe start by having a chat with your IT Security people and IT management around what policies you have around data access and related areas.
