cube process failed due to duplicate attribute key issue

    ERROR MESSAGE :

    "errors in the olap storage engine: table 'policy' ,column 'term' , value '1-ABCren',column 'policykey' ,value 'o1mj90',column 'quotekey' ,value 'osfa-201', The attribute is Term."

    my dimension hierarchy is having 4 levels and all are having compiste keys as key column.

    hieracrhy : policy -> Term -> episode ->section

    attribute relation is as : policykey->section->episode->Term->Policy

    please discuss the possibilities for this error occurance.
