There is a known bug with bulk insert and the role 'bulk insert admin': BUG: Cannot Perform BULK INSERT with Bulkadmin Privileges (302621) .
Even if the deny is removed, Mary would not be able to bulk insert. So 'never' surely was correct. She needs to be in the role 'db_ddladmin' or 'db_owners' to make it work.

I found the same problem. Upon trying out the scenario I found that Mary couldn't BULK INSERT into dbo.orders, even without the DENY in place.
I figured it must be a trick question and that the table was actually mary.orders. I based my answer on this assumption.
Given that this is a perfectly logical explanation, can I have my points back?
