Are there any known issues with the AMD Opteron 200 Series running Windows 2003 Ent. Edition and SQL 2000 Ent in a cluster?
In a few threads I have seen remarks that SP 4 is needed, but in the feature list of SP4 there is no mention about this.

Haven't seen any. SP4 had some AWE issues if you use that, so there was a patch for those.

SP4 adds support for Windows 2003 64bit. AWE patch should not be needed with 64 bit.

I'm in the initial phases of testing the Intel Zeon x64 running SQL 2000 EE and I'm not aware of known issues. I have had a few problems with 3rd party S/W support for Windows x64 (backup software, anti-virus). The readme file for SP4 explicitly states:
"SQL Server 2000 SP4 introduces support for SQL Server 2000 (32-bit) on Windows 2003 SP1 running in Windows-on-Windows 64 mode on X64 or compatible processors"
It doesn't make a distinction between AMD and Intel x64 chipsets. As for the AWE fix, since you are running SQL 2000 EE on x64 you still need the AWE hotfix. If you were running ia-64 you would not. This is stated on SP4 download page:

Don't forget: SQL2K 64 bit Edition runs ONLY on Itanium processor ie. NOT on AMD Opteron.
To be able run the 64 bit version you will have to upgrade to SQL2005
