Besides the Idera and Quest offerings, what else is out there that is worth auditioning?

Embarcadero looks way to involved to setup, and I'm not sure I like the idea of using agents.
Idera's SQL diagnostic manager seems like the way to go...

I would agree. I have found however some of the Idera tools have had some issues when interacting with a Clustered name instance of SQL server Enterprise addition. Never figured out why though (havn't tried to figure it out either though)
Other then that the software works pretty good. And is easy to use for DBA's and non DBA's

We are just starting to look at Microsoft System Center Operation Manager 2007 (MOM) So far its looks good but we are just beginning.

What features are you specifically looking for in a monitoring product, real-time monitoring, graphs & reports, notifications, etc ... ?
Do you have any sort of budget? Are there in-house skills to provide additional configuration/reporting/monitoring changes?

I evaluated several products and was impressed with "surprise" IBM.

The company did not go for the price tag and we implemented MOM instead. Microsoft Operations Manager has caused me a lot of pain, but I try to think that every monitoring tool will have a few issues. MOM seems to keep bringing new issues every now and then.
Also, it depends on your set of business requirements/budget/need like philcart was getting into...
