Hello all,
I am trying to do what is arguably the most boring part of a DBA's job, but totally necessary. We're experiencing growth, and it's time to start getting user requests via forms that a sponsor fills out and submits after the account has received approval. That way, I know what a user has access to, and what I need to change/delete/clean up when an employee leaves.
I'm not trying to reinvent the wheel, here. Does anyone have a set of forms that they find useful for tracking the things that we as DBAs have to track? User accounts are just one example. I have yet to start building my own repository of this kind of thing.

I've had to do the same thing in the past. However the best place to start for this information is generally the source. Who does the hiring/firing ? This is generally an extension of the HR process of mving people in and out of the organization. This gets you started. As for applications and access, well, now you're a part of their process. But this is a good thing, since legally, they are responsible !

In most cases, I would be happy to hear that. Exisiting documentation in-house is the best. However, I am a contractor who is subcontracted to a contractor who was contracted by the government. My company's HR has no resources for this, nor does that of the prime. My predecessors had so few users that they never bothered to create documentation.
So, I'll leave the request out there. If anyone has forms that could be useful as-is or that could be modified to fit the situation, I'd love to see them.
