Hi,
I have sp4 installed on one of my sql 2000 servers but sp3 on all the other sql 2000 servers.
Do you know of any issues that may develop? Have I made a serious mistake? Am I going to get fired ( again ).

SP4 has been out awhile. I don't read about many complaints (unlike SP3).

SP4 has been out for more than two years and as far I know is pretty stable

Well we had it removed as it was creating blocks and slowed down procedure execution.

There are some issues if you are running AWE extended memory on the server. Microsoft has some hotfixes that will take care of this. A search for SP4 and AWE at MSDN will fix you up. We've not noticed any performance degredation with sp4 -- after the hot fixes that allowed us to use the installed RAM.

I have also heard about some issues on SP4 but there had been a post SP4 cumulative Hotfix been published. I installed both and have not had any problems.

Well, with service pack 4 there is a "feature" affects us very hard. Read here:
