I've found that more often than not at work, the problems are more about communication than they are about technology. Your example is but one demonstration of this.

Even if you are a solo DBA, you need to communicate information about system changes -- with your future self! Without some kind of log or journal, will you remember what changes you made six months or a year from now?

6 months? I've struggled to remember things I did last week at times.

90% of the documentation I write is for me, for that same reason, Steve.
It's become a habit now, and not only does it save my own butt, it makes me look like a star when I can hand it over to someone else. 🙂

Even I forget the changes I made. A change log is essential whether it's one or more than one DBAs. It would be best if the log was the same one that IT keeps for system changes.
