Can a named instance be added to an existing 2 node active-passive cluster or do I need to create another virtual server?

Should be able to add it.

Do you want the new named instance to be clustered? If so, then yes - you need to add a new SQL Group, dedicated drives, etc...
If this new names instance will only exist on a single node of the cluster ever - then no.
