The article doesn't seem to render very well (at least when I'm viewing it in chrome). Perhaps you have some rogue HTML in it? At the moment it's hard to see the reasons the script is useful etc.

I have entered information in the fields provided , it is a simple plain text, not sure if it is an issue with the page by itself,
Nevertheless , the post is about creating a database using dynamic SQL, which picks up the location from the default path mentioned in the server configuration, now a simple CREATE DATABASE <database_name>; would also do that for you, however if you pass the file path in the variable declared for the data and log file location, the script should do the job for you and put the files where you want it to, Give it a go!!
