Of course, if there was a unique constraint on one or more columns of the table, the answer would have been 1 row inserted and 4 errors....

Is there a PK on the table on the pharmacy and/or claim no. If so you'd get one row inserted and 4 PK constraint violation errors.

Assume no PK if not mentioned, and yes, likely aiming for similar questions in the future with other variations.
