Yes.
If you followed best practice to allow data change by external applications only via stored procedures you could add the OUTPUT clause to the UPDATE statement together with the internal tables "inserted" and "deleted". Based on the result you can send the email using sp_send_dbmail.
If you change the data directly via your app you'll need to implement the logic as mentioned above in your update statement.
