Without a detailed description of your business requirments, plus proper definitions and descriptions of what each of those tables and columns actually represents I don't think anyone can answer you.
You may find someone can offer consultancy services to visit you for a few weeks and work through the design with you, but this is not really something that can be answered on a forum.

+1, completely agree a lot of the design wil depend on the business rules.
One thing you might want to consider is splitting Claims and Policies into seperate facts.
