  I am looking for a ready made template for change management. something off the shelve document.

  Database refresh

  User creations,

  INdex creations,

  Running scripts

  change security permissions etc.

  Customers needs to fill these templates and send it to the change managment board and then it goes to DBA.

  any help or links would be appreciated.
