I have SQL Server 2005 RTM version.
Shall i directly Update with SP4 or I need to go thorugh sp1,sp2,sp3 and sp4 ?

I've never had a problem patching to the latest SP personally, so I'd opt for going straight to SP4 IF it's applicable for you.
Always check the release notes with each SP to see if you utilise the functions patched. If your not using some of them it might be safer to just patch up to SP2 or 3. 😎

SP2 is old. SPs are cumulative. I suggest going directly to SP4, or SP3 at the minimum.

It's always safer to retest the whole app whenever possible.
Rolling back those upgrades is a major pita.
